Call 630/515-7233 to schedule an appointment.
Missed / Canceled Appointment Fees
- $10.00 – for any lab appointment that is not cancelled ahead of time or for canceling a physician appointment with less than 12 hours notice
- $25.00 – for failure to show up for a physician appointment
Immunizations and Titers
Before rotations, you must be in compliance by submitting all documentation for immunizations and titers from your certificate of immunity.
Immunization Requirements
You are responsible for periodically checking your immunization compliance status at http://online.midwestern.edu/. If you notice information you believe is incorrect, please stop by the Wellness Center to discuss it with us.
You are responsible for keeping copies of all immunization documents turned in to the Wellness Center. The Wellness Center will make only one copy of immunization documentation in your chart at no cost. For the second and all subsequent requests, you will be charged $5.00 each for a copy of any immunization document.
