As the new Chair, I welcome you to the 65th Anniversary of the Annual Ward E. Perrin, D.O. Clinical Refresher Course. The courses, designed to enhance clinical performance and continuing in Dr. Perrin's tradition, offer the latest updates in medical knowledge and applied workshops in osteopathic practice, all delivered by expert faculty of CCOM and distinguished guest speakers. In honor of the 65th year and looking to the future, the course offers system based case studies, choice of case based OMM track or a hospital procedure techniques track, Practice Management session, pharmacy updates and Elder Care sessions. We look forward to welcoming you to this exciting and enriching course.

Audience

This program was designed for Internal Medicine, Family Practice, Hospitalists, P.A.'s and other D.O. physicians seeking category 1-A AOA approved continuing medical education credit.

Faculty Disclosure

It is the policy of the CCOM CME program to ensure balance, independence, objectivity, and scientific rigor in all its educational programs. Individual activities are developed independently of educational grant support. In accordance with the standards of the AOA, commercial support of this meeting has had no influence over topics or speakers. Faculty disclosures and references to unlabeled or unapproved uses of drugs or devices will be available in the course syllabus and verbally announced during faculty introductions.

Physician Assistants

PAs may submit hours for a maximum of 22 credits. MWU/CHS PA alumni are invited to participate in all or any of the CME Program to meet with former classmates while continuing your education.

Registration Desk

Registration will be available at the McDonald's Campus on Thursday, Nov. 1, 2012 from 8:00 am through 1:00 pm and Friday and Saturday, Nov. 2-3, 2012, beginning at 7:15 am.

Annual W. Don Craske Memorial Lecture

The W. Don Craske Memorial Lecture will be presented during the luncheon on Saturday, November 3, 2012 from 12:00 noon until 1:00 pm. The honored guest presenter is George Caleel, D.O., MACOI.

Alumni and Student Mixer

Please join alumni and current CCOM students at the Hyatt Lodge for a complimentary mixer to be held Thursday, Nov. 1, 2012, from 6:00 pm - 8:00 pm. We look forward to seeing you there!

Annual CCOM Alumni Association Meetings & CCOM Alumni Association Awards

The Annual Meeting of the Alumni Association will take place on Friday, Nov. 2, 2012 from Noon until 1:30 pm during the luncheon. Please plan to attend this annual business meeting of your Alumni Association. The Annual Alumni Awards will be presented at the Friday luncheon. Join us to honor our 2012 Alumni Award recipients along with the induction of the Class of 1962 into the Gold Medallion Club.

Conference Location

65th Annual Ward E. Perrin, D.O. Clinical Refresher Course has two locations. The Thursday, November 1, 2012 sessions from 8:00 am to 1:10 pm will be at the McDonald's Campus , 2715 Jorie Boulevard, Oak Brook, IL (H.U. security ph. 630-623-7845) and from 1:40 pm to 5:10 pm at the Midwestern University Chicago College of Osteopathic Medicine, 555 31st Street, Downers Grove, IL (operator 630-969-4400). Friday and Saturday, November 2 and 3, 2012 will be held at the  McDonald's Campus.

Hotel Accommodations

A block of rooms has been reserved at The Hyatt Lodge at McDonald's Campus, 2815 Jorie Boulevard, Oak Brook, IL 60523 at a special rate of $ $125 single or double occupancy, $150 Triple occupancy, or $175 for Quadruple occupancy until 5:00 pm on October 10, 2012. After this date, or after the block fills, any reservations made will be based on room availability and subject to a higher rate. Guest rooms will not be booked by CCOM. Reservations can be made online at https://resweb.passkey.com/ go/MWUCRC or by calling 630-568-1234, request reservations, and the "Midwestern University" room block. All hotel cancellations are the sole responsibility of the guest(s) and must be made by 3:00 pm the day prior to arrival to avoid charge for the first night's room and tax. The departure day will be verified upon arrival; if departure date is changed after check-in, room charges will apply. If the guest(s) depart earlier than agreed, room charges for that day will incur. Departure extensions are based upon availability and must have prior approval or a late check-out charge of ½ the room rate is assessed if a guest checks out after 12:00 noon but before 3:00 pm. After 3:00 pm, a full group rate is charged. Payment to the hotel must be made in cash or on a major credit card. Personal checks will not be accepted. Check-in time is 3:00 pm; check-out time is 12:00 noon on the day of departure. Accommodations prior to check-in time cannot be assured.

Parking

Surface self-parking is complementary. Valet parking is available at $8.00 per day or $12.00 overnight.

McDonald's Campus 
The mandatory dress code for the McDonald's Campus requires professional, business casual attire and all guests are required to wear a printed name badge while at the McDonald's Campus.
