Increasing Performance Through Feedback

Providing and receiving performance feedback is essential to professional development. Managers have an important responsibility to deliver timely assessments.

  • Staff Reviews are administered electronically through UltiPro Performance Management. Human Resources provides UltiPro instructions for staff reviews.
  • A tool to assist with staff reviews is the Staff Self-Assessment Form. This form captures an employee’s point of view of performance from the previous assessment period. This is a voluntary form provided by HR, but managers have to option to make its completion mandatory for their staff.
  • Faculty Reviews are now on line through the CARS portal under the Main tab > Faculty Performance Eval or remain in hard copy forms. Please see the links to these forms below:

Staff Self-Assessment Form

For all Performance Review Forms, follow the instructions provided by HR during the Performance Review period. The Staff Supervisor and Staff Non-Supervisor process is administered through the online UltiPro Performance Management system.

Chair/Program Director Review

Compensation Request Form

Upon receiving the signatures for Manager and Manager's Supervisor, send form to the Director of Human Resources.

Excellence in Service Award

Nomination Form

Review the recommendations prior to completing the nomination form.

The University Service Award Committee periodically meets and reviews nominations for the Excellence in Service Award.

The web form will be sent directly to the appropriate IL or AZ Campus HR Representative.
