The College recognizes that a student may desire to be excused from class or rotation for non-illness, non-emergency-related reasons. An Absence Request Form must be completed at least two weeks prior to the day that the student wishes to be excused. Forms are available from the CPDG Dean's Office and at www.midwestern.edu. Completion of the form by the student does not imply the student is excused from classes until the course director of the affected courses approve the request.
