All assignments are to be documented in RMS and copies are to be placed in your portfolios. Your preceptor will certify that all your assignments are complete and acceptable before your final evaluation. Thereafter, your grade will be assigned. If your assignments are not done by the last day of your APPE, you will receive an incident report.
The following assignments are required for each APPE:
5 SOAP notes (minimum) -OR- 4 SOAP notes and 1 Self-reflective writing assignment) (patient care)
Case Presentation (patient care)
Evidence-Based Medicine Assignment (must have 2 over entire APPE sequence) - OR -
Article Review (must have 2 over entire APPE sequence)
Special Project (non-patient care elective only)
Final Evaluation (submitted by preceptor)
Supplemental Hours Form
Must have absence form for each absence (only 2 days allowed per APPE)

Please follow the links below for each of the assignments for your APPE rotation.
SOAP notes are required for each Patient Care APPE. You will be evaluated on at least 5 notes on pharmacotherapy interventions using information collected from patient interviews, medical records and patient profiles. 5 SOAP notes (minimum) -OR- 4 SOAP notes and 1 Self-reflective writing assignment.
You are required to present one patient case for each Patient Care rotation. This should be an expansion of one of the SOAP notes and include a discussion of patient information, drug therapy and a conclusion.
Although you are required to conduct two formal article reviews over the entire APPE sequence, you may not be assessed for more than one article per APPE, but should take advantage of the any opportunity to strengthen your skills. You can complete either an article review or an evidence-based medicine assignment and may not be assessed for more than one of these assignments during each APPE.

If you are completing a non-patient care elective you have two options: You can complete a special project or a reflective writing assignment.
This evaluation is help you self-assess your progress. You must complete this in PEMS so that the preceptor can complete their portion.
You must complete an evaluation of your preceptor/site on PEMS. A report will be sent to the preceptor anonymously at the end of the academic year.
All students must accumulate 36 (1440 hours) weeks over all APPEs. You will be required to complete 160 additional hours per ACPE standards as CPG currently provides only 1280 hours.
The final evaluation is pass or fail. If the student has unacceptably performed in any area, the student fails the rotation. The preceptor completes the final evaluation and reviews with the student. OEE submits the final grade to the registrar's office.
