Application

If you are interested in becoming a preceptor for Midwestern University's College of Pharmacy, Downers Grove Campus students, please fill out this Preceptor Profile form and forward as indicated. Within a few days of your submission, you will be contacted by the Office of Experiential Education.

Prospective Preceptor Resources

Why should I become a preceptor?
You may remember your first days on rotation or even starting at a new practice site, eager to learn and contribute, but a little hesitant and unsure of yourself. Perhaps a past preceptor or mentor explained, demonstrated and inspired you to think for yourself and achieve. Today, you have the opportunity to be that person by giving back to the profession. Becoming a preceptor and mentor to a student is one of the most honored contributions you can make to the profession!

The college values its partnerships with their pharmacists in the community and recognizes their efforts by offering the following benefits:

What are the requirements?
Pharmacists interested in joining the health care community at Midwestern University should have at least one year professional experience, maintain an active license, and be in good standing with their state board. Prospective preceptors must complete our Preceptor Profile form and preceptor training.

Introduction to Pharmacy Practice Experience (IPPE) I - Community
PPRAD 1691B
This course will allow students to gain an appreciation of the knowledge, skills, and socialization needed to practice in the community setting. Students will meet weekly to observe and discuss the role of the community pharmacist and participate in daily operations that focus on the distributive aspects of practice. Guided exercises and an introduction to the basics of community practice will serve as a foundation for advanced pharmacy practice experiences.

Introductory Pharmacy Practice Experience (IPPE) II - Health Systems
PPRAD 1692B
This course will provide students exposure to the fundamentals of pharmacy practice in a healthy system or hospital setting. The format will be a combination of didactic lectures, simulations, and on-site experiences. Students will gain practical experience with systems used to manage the medication use process and be introduced to drug distribution systems, pharmacy operations, sterile product preparation, medication reconciliation, medication safety, and drug information.

Community Advanced Pharmacy Practice Experience
PPRAD 1802
This practice experience will enable students to develop their skills and gain experience in a variety of patient-oriented services in community practice. In addition to medication dispensing, students will focus on drug therapy assessment and intervention, identification, resolution, and prevention of drug-related problems, education, and communication with patients and other healthcare professionals. Students may participate in additional services such as administration of immunizations, blood pressure screenings, and medication therapy and disease state management.

Hospital Advanced Pharmacy Practice Experience
PPRAD 1803
This practice experience will enable students to develop their skills and gain experience in hospital practice or other system of integrated pharmacy services. Emphasis is placed on systems to promote safe and effective medication use to optimize patient outcomes. In addition to medication order processing, preparation, and distribution, students will focus on drug-use decision-making processes, individual patient drug therapy monitoring, formulary management, and communication with patients and other healthcare professionals.

General Medicine Advanced Pharmacy Practice Experience
PPRAD 1804
This practice experience will enable students to develop their skills and gain experience related to general medicine in an acute care setting. Emphasis is placed on participating in an interprofessional healthcare team, drug therapy assessment, patient care activities, and monitoring of outcomes in various patient populations. In addition, students will participate in drug therapy interventions, identify drug related problems, develop therapeutic care plans, and effectively communicate with patients and other healthcare professionals.

Ambulatory Care Advanced Pharmacy Practice Experience
PPRAD 1805
This practice experience will enable students to develop their skills and gain experience related to ambulatory care practice. Emphasis will be placed on participating in an interprofessional healthcare team, and sharing responsibility with patients, caregivers, and other healthcare professionals to achieve optimal drug therapy outcomes. In addition, students will participate in drug therapy assessment and intervention, identify drug related problems, develop therapeutic care plans, and effectively communicate with patients and other healthcare professionals.

Clinical Specialty Advanced Pharmacy Practice Experience
PPRAD 1806
This practice experience will enable students to develop their skills and gain experience related to practice in a specific clinical specialty. Emphasis will be placed on participating in an interprofessional healthcare team, drug therapy assessment and intervention, patient care activities, and monitoring of outcomes in various patient populations. In addition, students will participate in drug therapy interventions, identify drug related problems. develop therapeutic care plans, and effectively communicate with patients and other healthcare professionals.

Elective Advanced Pharmacy Practice Experience
PPRAD 1807
This is an experience where students will gain an adequate breadth of knowledge, skills, and experiences specific to their chosen elective area. Pharmacy students may select from a list of electives with a variety of patient-care or non-patient care experiences such as an additional clinical specialty (when available), managed care pharmacy, academia, or pharmacy administration.

How do I access RMS?
You can find RMS on the internet at: online.midwestern.edu. You will need to begin with your login information.

I cannot remember my log in or password on RMS. Who can help me?
You can either contact anyone in the OEE  by calling 630-515-7677 Monday thru Friday between the hours of 8 a.m. and 4:30 p.m. Central Standard Time, or you can email cpdgoee@midwestern.edu.

How do I complete my student's mid-point evaluations?
Log into RMS > Click on the "Evaluations" tab > Locate your student > Click the "Mid-Evaluation" tab > Complete the evaluation and offer comments where indicated > Click submit

How do I complete my student's final evaluation?
Log into RMS > Click on the "Evaluations" tab > Locate your student > Click the "Final Evaluation" tab > Complete the evaluation and offer comments where indicated > Click submit

I'm on vacation. Who else can complete my student's evaluation?
A student's evaluation must be completed by someone who has been approved as a preceptor by Midwestern University, Chicago College of  Pharmacy, and has spent a significant amount of time with the student in order to grade him/her fairly. Please contact our office if someone other than the assigned preceptor will be completing the evaluation. If this person if not currently an approved preceptor, we will need a completed Preceptor Profile form and resume or CV.

Where can I find my student's contact information?
Log into RMS > Under the "Rotations" tab you will see all of the students assigned to you > Locate your student and click on the student's name.

How can I see a list of students assigned to me?
Log into RMS, and under "Rotations" you will see the rotation schedule. All of the students assigned to your site will be listed here. You can also click on the name of the student and/ or site to obtain additional information about the entity.

A preceptor has left our facility. Who do I tell?
Please contact our office and we will update the changes in RMS, usually within 48 hours. Please email cpdgoee@midwestern.edu for assistance or call 630-515-7677 for assistance Monday through Friday, 8am to 4:30pm.

A pharmacist has joined our facility and would like to become a preceptor. What do they do?
Please have the preceptor complete a Preceptor Profile form, and return it to us with a recent copy of his/her CV or resume. Our office will let you know as soon as the application has been approved, usually within 48 hours.

I am leaving my current site, but I still want to precept when I move. What do I do?
