Accreditation

Midwestern University College of Dental Medicine Arizona is an ADA CERP Recognized Provider.

ADA CERP is a service of the American Dental Association to assist dental professionals in identifying quality providers of continuing dental education.

ADA CERP does not approve or endorse individual courses or instructors, nor does it imply acceptance of credit hours by boards of dentistry.

Registration Information

There are two ways to register for our continuing education programs

Payment Methods

The College of Dental Medicine accepts checks, American Express, Diners Club, Discover, JCB, MasterCard and Visa.

Cancellations & Refunds

The Midwestern University College of Dental Medicine-Arizona Continuing Dental Education (MWUCDM-AZ CDE) office reserves the right to end registration when the enrollment limit is reached and to cancel, modify or alter content, faculty or location for any course if circumstances indicate such a change is advisable.  If the MWUCDM-AZ CDE finds it necessary to cancel a course, a full refund will be issued.

No travel expenses will be refunded to the registrant in the event of course cancellation or cancellation of any registration.  Full tuition refunds are issued if the participant cancels in writing two weeks prior to a course.  Cancellations processed during the two weeks prior to course date will be refunded tuition minus a late cancellation fee of $75.  No refunds will be issued for cancellations received three days prior to the day of the course or in the event of a no-show.  Select courses may have individually specific cancellation fees and polices.

A $50 fee will be assessed for all returned payments.  Returned payments include returned checks and dishonored credit or debit card payments.

**Please note that the EFDA Program has specific cancellation and refund policies.

Verification of Participation

Upon arrival, course attendees of MWU CDMA continuing education programs sign-in at the Conference Registration Desk located outside the conference room.

Based on the Attendance Sheet signatures, Verification of Participation forms will be issued at the conclusion of the program.  In accordance with AGD PACE and ADA CERP guidelines, the form will have the following information:

  1. Midwestern University College of Dental Medicine - Arizona approved provider status and ID number (AGD PACE);
  2. The date(s), location, and duration of the activity;
  3. The title of the activity and specific AGD subject codes;
  4. Educational methods used (e.g., lecture, clinical participation, self-instruction);
  5. Number of credit hours awarded (excluding breaks and meals);
  6. A course completion code for each educational activity;
  7. The name of the participant;
  8. Notice of opportunity to file complaints.

Participants pick up their own Verification of Participation Forms.  The CE program cannot release any Verification of Participation to anyone but the participant.

NOTE: What is the difference between a certificate and an Attendance Verification/Participation form (AVP)?  A certificate attests that the attendee has acquired a specific skill, specialty, or advanced educational status.  An attendance verification/participation (AVP) is a record of attendance or participation at a continuing education course/event.  Currently, MWU CDMA does not offer any courses that would result in a certificate. 

Course Location, Time and Meals

Course Check-in time and location will be listed on each registrant's course confirmation.  Your confirmation will also state if a meal is being served.  If you have a dietary restriction, please call or email us at least one week prior to the course.

Recordings & Photographs

Course participants are not allowed to record audio/video or photograph any time during a presentation without the express permission of the Office of Continuing Dental Education.

Notice to Participants

Midwestern University reserves the right to photograph or videotape participants during our continuing education courses.  By participating in or registering for a course, each participant agrees that we may, for no compensation;

  1. Photograph or videotape each course registrant
  2. Reproduce, distribute, display, use or publish such photographs or video images and likeness, with or without such participant's name, by any method for any lawful purpose, including for example, such purposes as education programs, publicity, illustration, recruitment, fundraising, advertising, and web content.   Such photographs and video images shall be the exclusive property of Midwestern University.

Conflict of Interest

Midwestern University College of Dental Medicine - Arizona, Office of Continuing Dental Education (MWUCDM-AZ CDE) provides a range of education programs for the dental professional. When Continuing Education Credits are offered, the program is to be strictly educational and by definition, non-promotional. The following guideline contains the procedures under which MWUCDM-AZ CDE will engage in educational activities:

  • Continuing education programs are educational and non-promotional.
  • Continuing education programs are created and implemented independent from the promotional influences of the commercial entities.
  • Generic names will be used for any discussion of a specific product, when possible, and there will be a balanced presentation of competing therapies and any limitations on the data.
  • Instructors will adhere to the ADA's Principles of Ethics and Code of Professional Conduct.
  • Instructors will present using Evidence Based Dentistry.
  • Instructors will disclose any potential Conflicts of Interest in the start of continuing education program.
  • Conflict of Interest can include stock ownership, current or past employment, paid consultant services, paid speaking engagements, membership on advisory boards or funded research activities.
  • MWUCDM-AZ CDE will separate commercial entities product displays from the learning environment and disclose to all the participants any unrestricted grants or sponsorship received for the continuing education programming.
  • There will be opportunity for debate in panel discussions and small group discussions within all continuing education programs.

BODEX Remediation Information

Midwestern University's College of Dental Medicine - Arizona (CDMA) Continuing Education Office offers one-on-one custom designed courses to fulfill mandated requirements for remediation as ordered by the Board of Dental Examiners in Arizona and other states.

All courses are taught by College of Dental Medicine faculty instructors during the week and on weekends.   Our office will provide the required documentation to the Arizona State Board of Dental Examiners both before and after the course is completed, as well as provide documentation for your records.   Additional information will be provided upon your inquiry.
