Beginning with the class matriculating in Fall 2027, Midwestern University's participating colleges/programs listed below agree to guarantee acceptances (seats) or interviews for qualified Arizona State University applicants.
Requirements for all Midwestern University Programs
As a condition precedent to receiving an interview for Midwestern University (MWU) Programs, Applicants must meet all the requirements set forth below before the academic year commences for the incoming class:
- Have received, or be on track to complete and provide proof of completion by matriculation at MWU, a baccalaureate degree from ASU for all MWU Programs except for Pharmacy and Veterinary Medicine; and,
- Have completed the minimum number of prerequisite courses (as stated in the MWU Catalog) in the prescribed subject areas at a regionally accredited college or ASU with a grade of "C" (or its equivalent) or better before the program begins; and,
- Have the ability to fulfill all technical standards for the desired program successfully; and,
- Demonstrate a good understanding of the desired profession and a sincere interest in a career in that field, including shadowing at least one (1) health care professional prior to admission; and,
- Program-related experiences that indicate sufficient exposure for candidates to make informed decisions about health professional careers; and,
- Exhibit personal integrity and sound moral character; and,
- Reflect a people/service orientation through community service or extracurricular activities; and,
- Abide by the MWU Drug-Free Workplace and Substance Abuse Policy; and,
- Meet all other MWU requirements, including those documented in the catalog, student handbook, and this Agreement.
Additional considerations
International students
- Must complete a minimum of thirty (30) semester hours of coursework in the United States. Of the thirty (30) semester hours, fifteen (15) hours must be in the sciences, six (6) hours must be in non-remedial English composition, and three (3) hours in Speech/Communication.
- Additional requirements for international students who have been accepted into Midwestern ASU Programs are included in the matriculation agreement. These requirements are in addition to any specific MWU Program requirement.
Final decisions
The final decision on acceptance for Applicants shall be up to the sole discretion of the Program Admissions Committee, subject to approval by the Dean and/or Program Director.
Limitations
- No Guarantee: This Admissions and Articulation Agreement does not guarantee admission to Applicants and does not imply that Applicants will receive favorable treatment from the MWU Programs. MWU reserves the right to deny Applicants admission.
- Non-Exclusive: The arrangements in this Admissions and Articulation Agreement are nonexclusive. MWU and ASU may enter into similar agreements with other entities.
- Other Colleges/Programs: This Admissions and Articulation Agreement does not apply to MWU colleges and programs not expressly identified herein, or
- Use of Intellectual Property: ASU and its affiliates will obtain written approval and authorization from the MWU Office of University Relations prior to using the MWU's name, marks, copyrights, or any other intellectual property, for any purpose. ASU shall provide drafts of all promotional materials to MWU for review and approval prior to use.
- No Third-Party Beneficiary: This Agreement is intended for the benefit of ASU and MWU only, and is not for the benefit of, nor may any provision hereof be enforced by, any other person or entity, including the Applicant.
Although we anticipate a long and mutually satisfying relationship between our two educational institutions, this agreement may be discontinued for any reason, provided that written notice is given twelve (12) months prior to such discontinuation and that those students who have been accepted under this agreement be permitted to complete their course of study.

In addition to the requirements set forth in section "Requirements For All MWU Programs" above, Applicants must also meet the following requirements to receive an acceptance or interview with CDMA and/or CDMI:
College of Dental Medicine-Arizona (CDMA)
Applicants must also meet the following requirements to receive an interview with CDMA and/or CDMI:
- Achieve a minimum of a 3.50 cumulative grade point average (on a 4.00 = A scale) in the respective Applicant's undergraduate coursework (with a 3.50 cumulative grade point average in sciences); and,
- 440 in each section of the DAT taken within the three years leading up to their anticipated matriculation at MWU CDMA.
- CDMA will reserve 5 acceptances. If by March 1st of the annual admissions cycle, MWU has not accepted at least 5 applicants from ASU, the MWU may, at its discretion, offer the remaining seats for that cycle to other applicants.
College of Dental Medicine-Illinois (CDMI)
Applicants must also meet the following requirements to receive an interview with CDMA and/or CDMI:
- Achieve a minimum of a 3.50 cumulative grade point average (on a 4.00 = A scale) in the respective Applicant's undergraduate coursework (with a 3.50 cumulative grade point average in sciences); and,
- 440 in each section of the DAT taken within the three years leading up to their anticipated matriculation at MWU CDMI.
In addition to the requirements set forth in section "Requirements For All MWU Programs" above, Applicants must also meet the following requirements to receive an acceptance or interview with CHS-Glendale and/or CHS-Downers Grove.
Cardiovascular Science Program - Glendale, AZ
- Achieve a minimum of 3.33 cumulative grade point average (on a 4.00 = A scale) and a 3.21 average science GPA in their undergraduate coursework.
- Submit a minimum of two (2) letters of recommendation from professionals who know the applicant well.
- Complete an online Application for Advancing Health Professions application by September 1st; and
- CVS will reserve 6 acceptances. If by February 1st of the annual admissions cycle, MWU has not accepted at least 6 applicants from ASU, the MWU may, at its discretion, offer the remaining seats for that cycle to other applicants.
Clinical Psychology - Downers Grove, IL
Applicants must also meet the following requirements to receive an interview:
- Achieve a minimum of a 3.50 cumulative grade point average (on a 4.00 = A scale) in the respective Applicant's undergraduate coursework; and,
- Must complete an online application at the Centralized Application Service for Graduate Psychology (PSYCAS) website by May 1st; and,
- Submit a minimum of two (2) letters of recommendation from professionals who know the Applicant well to PSYCAS; and,
- Provide to PSYCAS official transcripts from every undergraduate, graduate, or professional school attended. Each transcript MUST be signed and sealed by the registrar of each institution.
Clinical Psychology - Glendale, AZ
- Achieve a minimum of a 3.50 cumulative grade point average (on a 4.00 A scale) in the respective Applicant's undergraduate coursework; and
- Complete an on line application at the Centralized Application Service for Graduate Psychology (PSYCAS) website by January 15th.
- Submit a minimum of two (2) letters of recommendation from professionals who know the Applicant well to PSYCAS; and,
- Clinical Psychology in Glendale will reserve 6 acceptances. If by February 1st of the annual admissions cycle, MWU has not accepted at least 6 applicants from ASU, the MWU may, at its discretion, offer the remaining seats for that cycle to other applicants.
Nurse Anesthesia - Glendale, AZ
The Nurse Anesthesia Program may guarantee interviews to ten percent (10%) of applicants from a participating partner institution's nursing program. Because admission to nurse anesthesia programs requires prior intensive care unit experience as a registered nurse, eligibility criteria for a guaranteed interview differ from those of other Midwestern ASU programs.
Requirements to obtain a guaranteed interview include:
- Achieve a minimum overall cumulative grade point average of 3.30 (on a 4.00 =A scale); and,
- Achieve a minimum cumulative science grade point average of 3.75; and,
- Hold Critical Care Registered Nurse (CCRN) certification; and,
- Complete a minimum of one (1) year of full-time critical care registered nursing experience prior to application. Acceptable critical care experience includes Adult Intensive Care Units (ICU), Pediatric Intensive Care Units (PICU), and Flight Nursing. Neonatal Intensive Care Unit (NICU) and Emergency Department (ED) experience do not meet this requirement. Qualifying experience should include management of mechanical ventilation, invasive monitoring, and vasoactive medication infusions.
Occupational Therapy - Downers Grove, IL
Applicants must also meet the following requirements to receive an interview:
- Achieve a minimum of a 3.50 cumulative grade point average (on a 4.00 =A scale) in the respective Applicant's undergraduate coursework; and,
- Must complete an online Centralized Application Service for Occupational Therapy Program (OTCAS) application by September 1st; and,
- The Applicant should refer to the OTCAS application instructions for specific details about completing the application, required documents, and processing time.
- Must submit a minimum of two (2) letters of recommendation directly to OTCAS. The Office of Admissions will only accept letters of recommendation received directly from OTCAS. It is preferred that one (1) of the submitted letters is written by an occupational therapist who has supervised or mentored the Applicant or is a professional who can speak to the Applicant's motivation, experiences in occupational therapy or readiness for entering the Occupational Therapy Program. The second letter can be written by either a college professor who taught the Application or a pre-health advisor who knows the Applicant well.
- The Applicant should refer to OTCAS application instructions for specific guidelines and requirements for submitting letters of recommendation.
Occupational Therapy - Glendale, AZ
- Complete a baccalaureate degree from a regionally accredited college or ASU; and
- Achieve a minimum cumulative undergraduate grade point average of 2.75 (on a 4.00 scale); and
- Complete all prerequisite coursework with grades of C or better (C- is not acceptable); and
- Complete the following prerequisite courses at a regionally accredited institution:
- Human Anatomy (3 semester/4 quarter hours), Physiology (3 semester/4 quarter hours), Statistics (3 semester/4 quarter hours), Human Development (3 semester/4 quarter hours), Abnormal Psychology (3 semester/4 quarter hours), and one additional Social and Behavioral Science course (3 semester/4 quarter hours); and
- Satisfy the Anatomy and Physiology requirement, which may be fulfilled by completing Anatomy and Physiology I and II if offered as combined courses; and
- It is strongly recommended that Human Anatomy be completed within five (5) years prior to admission, and that a lab component including cadaver experience be completed; and
- Submit coursework that may include alternative coursework in basic sciences, psychology, anthropology, mathematics, research, human development, or human movement, which will be evaluated by the Occupational Therapy Program Admissions Committee to determine whether such coursework, along with relevant work or experiential learning, fulfills prerequisite requirements; and
- Submit a minimum of two (2) letters of recommendation; and
- Successfully complete the Occupational Therapy Program's interview process; and
- Agree to comply with Midwestern ASU's Drug-Free Workplace and Substance Abuse Policy; and
- Pass a criminal background check (noting that a felony conviction may impact eligibility to sit for the NBCOT Certification Examination or to obtain state licensure).
- Will reserve 14 acceptances, If by February 1st of the annual admissions cycle, MWU has not accepted at least 2 applicants from ASU, the MWU may, at its discretion, offer the remaining seats for that cycle to other applicants.
Physical Therapy - Downers Grove, IL
Applicants must also meet the following requirements to receive an interview.
- Achieve a minimum of a 3.50 cumulative grade point average (on a 4.00 =A scale) in the respective Applicant's undergraduate coursework (with a 3.10 cumulative grade point average in sciences); and,
- Must complete an on-line application to Physical Therapist Centralized Application Services (PTCAS) by September 1st; and,
- The PTCAS application includes instructions for specific details about completing the application, required documents, and processing time.
- Must submit a minimum of three (3) letters of recommendation directly to PTCAS. It is preferred that one (1) letter is written by a licensed physical therapist. The other two (2) letters can be written by pre-health advisory committee, pre-health advisor, or a college professor who knows the Applicant well.
- Applicants should refer to the PTCAS application instructions for specific guidelines and requirements for submitting letters of recommendation.
Physical Therapy - Glendale, AZ
- Achieve a minimum of a 3.40 cumulative grade point average (on a 4.00 scale) in the respective Applicant's undergraduate coursework (with a 3.10 cumulative grade point average in sciences); and,
- Complete an online application to Physical Therapist Centralized Application Services (PTCAS) by March 15th (the PTCAS application includes instructions for specific details about completing the application, required documents, and processing time).
- Will reserve 10 acceptances. If by February 1st of the annual admissions cycle, MWU has not accepted at least 2 applicants from ASU, the MWU may, at its discretion, offer the remaining seats for that cycle to other applicants.
Physician Assistant - Downers Grove, IL
In addition to the requirements set forth in section "Requirements For All MWU Programs" above, Applicants must also meet the following requirements to receive an interview with Downers Grove PA program.
- Achieve a minimum of a 3.60 cumulative grade point average (on a 4.00 scale) in the respective Applicant's undergraduate coursework (with a 3.70 cumulative grade point average in sciences);
- Complete an online application in the Centralized Application Service for Physician Assistants (CASPA) by September 1st; and
- Submit a minimum of two (2) letters of recommendation from professionals who know the applicant well.
Physician Assistant - Glendale, AZ
- Achieve a minimum of a 3.70 cumulative grade point average (on a 4.00 = A scale) in the respective Applicant's undergraduate coursework (with a 3.70 cumulative grade point average in sciences).
- Complete a minimum of 500 patient contact hours.
- Will hold 20 guaranteed interview spots for ASU students
Speech Language Pathology - Downers Grove, IL
- Achieve a minimum of a 3.0 cumulative grade point average (on a 4.00 scale) in the respective Applicant's undergraduate coursework (with a 3.00 cumulative grade point average in sciences);
- Complete a Centralized Application Service for Communication Science & Disorders Programs (CSDCAS) application by February 1st; and
- Provide two (2) letters of recommendation from individuals who can comment on the academic, clinical, and professional experiences of the Applicant.
- Will reserve 10 acceptances. If by February 1st of the annual admissions cycle, MWU has not accepted at least 2 applicants from ASU, the MWU may, at its discretion, offer the remaining seats for that cycle to other applicants.
Speech Language Pathology - Glendale, AZ
- Achieve a minimum of a 3.0 cumulative grade point average (on a 4.00 scale);
- Complete a Centralized Application Service for Communication Science & Disorders Programs (CSDCAS) application by February 1st; and
- Participate in the program interview day receiving a favorable recommendation from the faculty interviewers.
- Will reserve 10 acceptances. If by February 1st of the annual admissions cycle, MWU has not accepted at least 2 applicants from ASU, the MWU may, at its discretion, offer the remaining seats for that cycle to other applicants.
In addition to the requirements set forth in section "Requirements For All MWU Programs" above, Applicants must also meet the following requirements to receive an acceptance or interview with AZCOPT and/or CCO:
Arizona College of Optometry (AZCOPT)
- Must meet all admissions requirements of either AZCOPT and/or CCO for which the applicant is applying; and,
- Achieve a minimum of a 3.25 cumulative grade point average (on a 4.00 scale) in the respective Applicant's undergraduate coursework; and,
- Complete the Optometry Admission Test (OAT) within the last five (5) years with a minimum Academic Average and Total Science scores of 300; and,
- Must complete an online application at the Centralized Application Service for Optometry (OptomCAS) website by April 1st.
- AZCOPT will reserve 4 acceptances. If by February 1st of the annual admissions cycle, MWU has not accepted at least 2 applicants from ASU, the MWU may, at its discretion, offer the remaining seats for that cycle to other applicants.
Chicago College of Optometry (CCO)
In addition to the requirements set forth in section "Requirements For All MWU Programs" above, Applicants must also meet the following requirements to receive an interview with AZCOPT and/or CCO:
- Must meet all admissions requirements of either AZCOPT and/or CCO for which Applicant is applying; and,
- Achieve a minimum of a 3.25 cumulative grade point average (on a 4.00 = A scale) in the respective Applicant's undergraduate coursework; and,
- Complete the Optometry Admission Test (OAT) within the last five (5) years with minimum Academic Average and Total Science scores of 300; and,
- Must complete an online application at the Centralized Application service for Optometry (OptomCAS) website at by April 1st.
In addition to the requirements outlined in section "Requirements For All MWU Programs" above, Applicants must also meet the following criteria to receive an acceptance or an interview with AZCOM and/or CCOM.
Arizona College of Osteopathic Medicine (AZCOM)
- Achieve a minimum of a 3.50 cumulative grade point average (on a 4.00 scale) in the respective Applicant's undergraduate coursework (with a 3.60 cumulative grade point average in sciences); and
- Receive a score of 507 or higher on the Medical College Admissions Test (MCAT) within the three (3) years prior to anticipated matriculation at MWU; and,
- Must submit a letter of recommendation from a D.O. or M.D. unrelated to the applicant.
- For admission, the applicant must also be recommended for admission by interviewer(s).
- Complete an online application at the American Association of Colleges of Osteopathic Medicine Application Services (AACOMAS). More information can be found at the American Association of Osteopathic Medicine.
- AZCOM will reserve 10 acceptances. If by March 1st of the annual admissions cycle, MWU has not accepted at least 10 applicants from ASU, the MWU may, at its discretion, offer the remaining seats for that cycle to other applicants.
Chicago College of Osteopathic Medicine (CCOM)
Applicants must meet the following requirements to receive an interview with CCOM:
- Achieve a minimum of a 3.50 cumulative grade point average (on a 4.00 = A scale) in the respective Applicant's undergraduate coursework (with a 3.60 cumulative grade point average in sciences); and,
- Receive a score of 504 or higher on the Medical College Admissions Test (MCAT) within the three (3) years prior to anticipated matriculation at MWU; and,
- Complete an on-line application at the American Association of Colleges of Osteopathic Medicine Application Services (AACOMAS). More information can be found at the American Association of Osteopathic Medicine.
In addition to the requirements set forth in section "Requirements For All MWU Programs" above, Applicants must also meet the following criteria to receive an acceptance with CPG and/or CPDG:
College of Pharmacy- Glendale (CPG) and College of Pharmacy, Downers Grove (CPDG)
- Achieve a minimum of a 3.00 cumulative grade point average (on a 4.00 scale) in the respective Applicant's undergraduate coursework; and,
- Must complete an online application at the Pharmacy College Application Service (PharmCAS) website by April 1 for the summer enrollment (June).
- CPG will reserve 15 acceptances. If by February 1st of the annual admissions cycle, MWU has not accepted at least 15 applicants from ASU, the MWU may, at its discretion, offer the remaining seats for that cycle to other applicants.
In addition to the requirements set forth in section "Requirements For All MWU Programs" above, Applicants must also meet the following requirements to receive an interview with AZCPM:
Arizona College of Podiatric Medicine (AZCPM)
- Achieve a minimum of a 3.50 cumulative grade point average (on a 4.00 scale) in the respective Applicant's undergraduate coursework (with a 3.60 cumulative grade point average in sciences); and,
- Receive a score of 495 or higher on the Medical College Admissions Test (MCAT) within the three (3) years prior to anticipated matriculation at MWU; and,
- Must complete an online application at the Centralized Application Service for Podiatry (AACPMAS) website by May 1st.
- AZCPM will reserve 3 acceptances. If by January 1st of the annual admissions cycle, MWU has not accepted at least 3 applicants from ASU, the MWU may, at its discretion, offer the remaining seats for that cycle to other applicants.
In addition to the requirements set forth in section "Requirements For All MWU Programs" above, Applicants will also meet the following requirements to receive an interview with CVM:
College of Veterinary Medicine (CVM) - Glendale, AZ
- Achieve a minimum of a 3.40 cumulative grade point average (on a 4.00 scale) in the respective Applicant's undergraduate coursework; and,
- Must complete an online application at the centralized Veterinary Medical College Application Service (VMCAS) website by September
- Must submit a minimum of three (3) letters of recommendation from professionals who know the Applicant well to VMCAS, one (1) letter must be provided from a veterinarian; and,
- Provide to VMCAS official transcripts from every undergraduate, graduate, or professional school attended. Each transcript must be signed and sealed by the registrar of each institution.
- CVM will reserve 2 acceptances. If by February 1st of the annual admissions cycle, MWU has not accepted at least 2 applicants from ASU, the MWU may, at its discretion, offer the remaining seats for that cycle to other applicants.
